This past week I was at a vendor who had an ipad mini with custom review app setup on the checkout desk.
of course i started playing with it right away just to pass the time.
the app was whack, I think it required 12 fields just to "sign up" and then took me through this long process where I had to fill out more fields. it was more like a customer satisfaction survey.
This was at an urgi-care type of environment so most of the people wanted to get in and out as soon as possible. From what I could see it was there for about a month and there were no reviews.
I've found that a card next to the cash register / whatever that says, "we're on Yelp" helps a lot. It does not ask for reviews but it will add enough co-branding to the point that you might remember to leave a good experience next time you are online.
